Sleep Diary

What is a Sleep diary?
The purpose of a sleep diary is to identify a person’s sleep and wake routine that may influence sleep quality and daytime sleepiness. Sleep diaries typically contain self recorded data for 2 weeks. The diary includes documenting bed time routine, approximate sleep onset times, awakenings at night, awakening in the morning, naps during the day, time of meals, use of medications, caffeine, alcohol, smoking, time of exercises, and stressors. The diary is logged by the patient and is useful in involving the patient in the diagnostic and treatment plan during the office visit.
